Case 1 of 8

Lingering pain to cold in a lower molar

The case

A 35-year-old woman has had pain in the lower right back teeth for 4 days. Cold drinks cause sharp pain that lingers for about a minute, and the pain woke her last night. She cannot say which tooth it is. She is healthy with no medications or allergies. The lower right first molar has a large, recent composite. Cold test: the first molar gives intense pain lingering 45 seconds; the second molar and premolar respond briefly and normally. Percussion and palpation are normal. The periapical radiograph shows a deep restoration close to the pulp and normal periapical tissues.

  1. Step 1 of 4What is your pulpal and apical diagnosis?

Think it through, ideally out loud, before you look.
