Case 1 of 8

Exposed bone after denosumab

The case

A 67-year-old woman with breast cancer metastatic to the spine is referred for a sore spot in the lower left jaw. Her oncologist started denosumab 120 mg every 4 weeks two years ago. A lower left first molar was extracted by her general dentist 3 months ago. She has had exposed, yellow-white bone in the socket since then, now about 1 cm across, with redness, tenderness and pus from the margin for the past week. She has no history of radiation to the head and neck. Other medications: letrozole and a daily vitamin D supplement. She is afebrile.

  1. Step 1 of 5What is your working diagnosis, and how do you confirm it fits the definition?

Think it through, ideally out loud, before you look.
